The patient portal also known as Health365 is a centralised health management system which allows patients the flexibility to book from wherever, whenever.  Helath365 gives our patients access to book appointments online, order prescriptions and view results. 

 

You will need to have your mobile number and email address recorded with us to enable you to complete the registration process. If you are a current patient and would like to join Health365 please email or call our reception to assist and give you the form to complete in order to activate your account or you can fill in the forms below and email them to reception@aucklandfamily.co.nz
